For your convenience, we require scheduled appointment times for our patients. We encourage these appointment times so that we can see you in a timely manner, focus on your needs, and develop a medical treatment plan specific to your condition or concerns. You may contact our office at (318) 212-3830 should you have a question about your appointment. If you are unable to make your scheduled appointment time, please contact us as soon as possible so that we can allow another patient to have this time slot.
You can also Request an Appointment Online.
We like for our New Patients to arrive 15 to 20 minutes prior to your appointment time so that we are able to process your information, create your medical file and place your chart in line to be seen at your designated appointment time. We often mail the following forms to you just to save time, but for those who have access to a computer and printer, you may download the required forms and arrive about 10 minutes early.
** Suggested FAA Medical Certificate applications can be assessed and completed online at FAA.gov. Please be sure to bring a copy of the printout or the confirmation number so that we can complete your certification in a timely manner.

However, your agreement with your Insurance Company is strictly between you and them so any remaining balance will be your responsibility and due immediately. You will also be responsible for payment should your insurance plan deny charges for any reason. Please contact your insurance carrier for questions regarding payment for any services rendered.
