Appointment Scheduling

For your convenience, we require scheduled appointment times for our patients. We encourage these appointment times so that we can see you in a timely manner, focus on your needs, and develop a medical treatment plan specific to your condition or concerns. You may contact our office at 318-212-3830 should you have a question about your appointment. If you are unable to make your scheduled appointment time, please contact us as soon as possible so that we can allow another patient to have this time slot.

Required Patient Forms

We like for our New Patients to arrive 15 to 20 minutes prior to your appointment time so that we are able to process your information, create your medical file and place your chart in line to be seen at your designated appointment time. We often mail the following forms to you just to save time, but for those who have access to a computer and printer, you may download the required forms and arrive about 10 minutes early.

** Suggested FAA Medical Certificate applications can be assessed and completed online at FAA.gov. Please be sure to bring a copy of the printout or the confirmation number so that we can complete your certification in a timely manner.

Fees & Billing

The Physician will determine your charges based on the level of service, any laboratory or x-ray testing, and/or any injections. To ensure that we file your claim to the correct insurance carrier, we ask that you provide a current copy of your insurance card at each visit.

We accept Cash, Check, MasterCard, Visa and Discover.

Pay Your Bill Online

You can pay your bill online using Online Bill Pay.

Insurance

We accept most insurance plans, so please ask the receptionist when you schedule your new appointment. As mentioned above, please provide us with your current insurance card so that we file your claim to the correct insurance carrier. We will file all charges to your insurance plan. However, your agreement with your Insurance Company is strictly between you and them so any remaining balance will be your responsibility and due immediately. You will also be responsible for payment should your insurance plan deny charges for any reason. Please contact your insurance carrier for questions regarding payment for any services rendered.

Release of Information

We are unable to release any information to anyone without your written consent on the required Alternate Contact Form. Your information is privacy-protected.
